Parish Registration Form

Below is the parish registration form for each of the three parishes. Select which parish you are registering for and fill out the card. You can then send it electronically via out parish email, or print it and place it in the collection basket at Mass. 

Youth Ministry Forms

For each event the youth attend, the diocesan policy requires them to have paperwork filed with the Youth Ministry office. If you would like to have your child attend an event, please print the necessary forms and fill them out prior to returning them to Cheyenne.

Youth:
Only needs one Form A on file for one year June 1 through May 31 the following year. 
 
​​A new Form B is needed for each event. If you have any questions call Cheyenne.
 
 
Adults: 
As chaperones or volunteers you have to fill out the form D, which is good for one year June 1-May 31. If you have any questions please call Cheyenne.
